How to Sign & Return

  1. Download the PDF and complete the signature fields (digitally or by hand).
  2. Sign in to your Healthie portal and upload the signed document to Documents or send via Secure Messages.

Please read and sign to acknowledge your understanding of the office cancellation policy and associated fees.

Rescheduling & Cancellation Window

If you need to cancel or reschedule your appointment, please provide at least 24 hours’ notice via email or text message, sent no later than 24 hours before your scheduled appointment time. You can reach us at kelly@myvenuscollective.com or (508) 356-4966.

Late Cancellations & No-Shows

While fees may be waived under certain circumstances, this is not guaranteed. Repeated late cancellations or no-shows may affect your ability to schedule future appointments with the office.

Refunds

Running Late

If you are running late, we will do our best to accommodate within your scheduled time. Significant delays may require rescheduling and may be treated as a late cancellation.

Provider Delays or Cancellations

If we must reschedule due to provider availability or technical issues, you may choose a new time or request a full refund of any prepaid amount.
